Piles is a disease caused by the formation of hemorrhoids or lumbs. the treatment and care of the disease depends on the severity of the problem .if the pain is mild it can be treated by home treatment without even going to a doctor using over the counter medications, ointments or pads containing some kind of anti inflammatory agent along with daily warm baths. but if the pain is too much of a problem and you are not being able to treat it with off the counter medications you might need to see a doctor and might even need a surgery. if there is a blood clot in an external hemorrhoid it can easily be removed with a simple incision which provides for instant relief.

Normally surgery is the last option for a doctor . the doctor normally tries to cure the patient using medications and ointments if the condition improves than its fine than a more serious procedure needs to be performed.

The next option which a doctor normally goes for is one of the following
1. Banding hemorrhoids: In this method the doctor places one or two rubber bands along the base of the internal hemorrhoid in order to cut off the circulation. as a result sooner or later hemorrhoids fall off. This simple procedure — called rubber band legation — is done in the doctor's office and is effective for many people.

2. Sclerotherapy: In this method a chemical solution is injected into the blood vessel to shrink the hemorrhoid so that it can easily be removed after that.

3. Infrared light: Here the doctor uses a 1- or 2-second burst of infrared light which cuts off circulation to small, bleeding, internal hemorrhoids.

If condition is far more critical to be solved by the above methods than surgery is the last option to be undertaken. normally if only it’s a critical cases when you have a very large hemorrhoids you might need to have a surgery where the doctor removes the whole tissue in a process called hemorrhoidectomy. Normal this surgery is done using local anesthetic combined with sedation, a spinal anesthetic, or general anesthetic. It's normally requires an outpatient or overnight hospital stay. although the surgery might be painful it is the most comprehensive method and medicines might be used to decrease the pain.
